The design features a marathon runner on the left and a detail of St. Elisabeth's Cathedral, one of Košice's architectural landmarks, on the right. The name of the issuing country, SLOVENSKO, is inscribed on the lower part, followed below by the year the marathon was inaugurated, 1924, and the year the coin was issued, 2024. The inscription KOŠICKÝ MARATÓN runs along the edge of the upper half of the design. Next to the lower left edge is the mint mark of the Kremnica Mint (Mincovňa Kremnica), consisting of the letters MK set between two corners. To the right of this are the stylized initials LR, in reference to the designer of the national face, Roman Lugár. The outer ring of the coin bears the 12 stars of the European flag.
